1. Wallet address
2. A detailed description of the issue you are facing.

3. Link of the Transaction hash (If exists)

4. Name and amount of the token

5. The exact action intended and the result, properly descriptive

6. Screenshots or screen recordings of the action

7. Name of the wallet
